I think that some form of demilitarizing should be a peace condition. Maybe demolishing forts, limiting army and navy size, etc. Also, war reparations should probably be a slider.

No, it's definitely not. An example off the top of my head is the Rush-Bagot treaty between the United States and Britain that limited navies on the Great Lakes. Also the Treaty of Munster, the Treaty of Rastatt, and the Treaty of Paris.
The war rep slider i Agree 100% on. The rest NOPE sorry, i don't think the AI would know how to use this properly. They would might do it, but they would not know when it would be smart to take land, or demolish forts. And why choose this peace option, when you just can take land.
And in Multiplayer, this would just be terrible.
