I'm no modder by any means but I am throwing out some thoughts I had while playing some Dark Souls the other day on how the setting and lore would work well as a CK2 mod.

The Black Death and epidemic mechanics could be used as hollows overtaking a territory. The Crusades being undead hunts to take back provinces and kill off undead and hollows. There could be event chains to send a character to rekindle a flame, only to have them fail, go hollow before completing the quest, or turn away and become a lord of dark. Not linking the flame would cause events of time and universe breaking down, abyssal darkness spreading, monsters, and insanity. Also there's magic, souls, humanity, armor, weapons, and items that all could be implemented with artifacts and traits.

Anyone else thought of something similar?

I think the main limitation would be an issue of scale. Outside Anor Londo itself we don't know much about what the world looks like, and what we see in it is essentially a post-apocalyptic wasteland abandoned by both God/Lord and mortal alike. A CK2 mod typically requires an actual political landscape, and that's all either relegated to the background far, far away, or centuries in the past.

It'd certainly interesting if done well, but that'd require a significant amount of effort devoted to world-building on the mod-creator(s) part.

That being said, I wouldn't mind playing Gwynevere, God-Empress of the Empire of Anor Londo.

I thought I remembered seeing this thread. Thought about it while playing Bloodborne, somewhat ironically.

It's true a massive amount of world-building would need to be done, because even on the scale of in-game locations, there are few "rulers" of any sort. DSIII has, what, 5-6 named hierarchal bosses in the entire kingdom? Granted, the "kingdom" at the scale we see is more of a city state (which might be an interesting idea on its own as well, ie boroughs, mayors, barons, etc) and there could be many other constituent parts we do not see because they are not directly relevant to the plot. Even more complicating is the fact that games don't really "branch out" the factions, they've almost all arisen on top of each other after immeasurable time has passed through the cycles, or are so far apart in addition to the time passed as to not interact with each other. There are comparatively few "dynasties" of any sort, with only a handful of CoA lying around.

I do wonder if a non-extensive overhaul mod would to just make all characters lunatic cynics and give them, at most, two virtues and every other sin, and immortal. Character bloat would be horrendous.

That being said, I would also like to see an extensive SoulsBorne mod come about. The universe always calls me back, so the more places that is, the better.
