Uhh, I'm not sure if I hit a bug or what but...
So I started a game as a random count of Lucca using a custom starting Dynasty name as Medici, of course, intending to become Tuscany/Florence. The problem is that I can't become a republic.
I've tried dozens of weird ways over the past two or three hours to try and get this to work but nope. I made my original guy move duchies and swap primary titles, but can't seem to give my now ex heir all the land in Tuscany or even Pisa. There are some cases where I can give Pisa (the whole duchy) but can't give anything in Tuscany. What I'm doing is I give my heir a City, gave the now Mayor a county, and the Lord Mayor a Duchy (though, sometimes I can't do this for some reason). I've tried just giving him a county city (like Pisa or Siena) and then giving him a bunch of cities. Nope. Blah I'm too tired its 6 AM.
Oh, there was one time when I did succeed but then when I went to reload into it it said 'I can't play Republics!' when I have the DLC on and can even play a different Republic I also released under a different guy (Genoa). Is there some rule that the leader of a Republic can't own any Baronies (including county baronies) or something?
Is the idea to destroy the Tuscany title, move primary title + capital to Spoleto, transfer titles, then reload? Bah I can't think. I think I even tried that.
