There's a beta patch if you feel the need to go for it and can't wait for it to properly be released for whatever reasons. Just click the properties off steam and delve through the menus and it should be reasonably apparent how to grab it. Not sure how much longer they'll want to test it or what issues they're still trying to fix up before rolling it out officially, though.
I have a republic question too. I had southern Italy as ERE for a long time, but then I decided to create the duchy of Napolis. When I did that (I gave it to the count of Gaeta), I got the notification of a great house rising and he became the Doge of Napolis. So he turned into a vassalised merchant republic as I understand it right?
Is this good or bad?

I can't remember if this is supposed to happen: I holy warred for the 2 highlighted regions below (the duchy of Saxony which was owned by an independent slavic ruler). The actual duchy title belonged to someone in the HRE who owned all of the de jure area of the duchy of Brunswick (but the Brunswick duchy title didn't exist). So, now that I own 2/3 of the duchy of Saxony, I usurp it and steal every single vassal count the former Duke of Saxony had (9 counts in total as you can see by the shields).
Is this meant to happen? Or is this one of the dangers of owning none of the de jure counties that are associated with your title while having no 2nd title of equal rank to it?

Thank you. I guess that means this was a really bad screw up by the AI... so I gained 11 counties for the price of stomping a 500-man army count. I've actually never had this happen to me before, all I ever usually got from usurping was the title itself.
Actually, I don't think that's supposed to happen. Yeah, the former Duke becomes a Count and thus loses all his Count vassals but from what I know they all should have stayed within the HRE.
If a neighboring ruler has a high opinion of you is he less likely to start de jure claim wars on you? It seems like most of my defeats are because someone believes that something rightly belongs to them. Is there a way to keep them from grabbing your lands?

Offhand, I would say yes, as the decision is now taken out of your hands. Whether it actually happens or not I can't say.
