It's a OR. The AND reference to the next column.I noticed something that seems odd in landed_titles.txt. This code appears to require you to not have k_france but also to have k_france. Am I reading this correctly? Should I go make a bug report?

Which I haven't, I dismissed all personal and vassal levies, even disbanded all my retinues.
China is currently Open and Stable, maybe that is the cause?

I have been trying to play a Venice game for some time now. It's impossible. Everytime some raider shows up and my trade posts get destroyed. Since I like to build the first one in Venice one would assume the doge would raise an army, but nope. They siege down the capital and the TP gets destroyed along with it.
I managed to build the garrison upgrade, but just a few years into the game, they show up wit 1000+ raiders. That ain't fun.

To elaborate though, whenever you see an achievement that calls for starting in 1066 it always means the original earliest bookmark, the Stamford Bridge start. I believe the reason that was made a thing is because a few of the oldest achievements would be too easy if you started as William following the Norman Invasion.To get the 1066-1453 achievement do I have to start in September 1066 or December 1066?
