No, moral authority is not a factor. Some religions lack the holy war CB (unreformed Pagans, Jains, and technically Buddhists did too) so maybe that was it (11% MA does sound like you might have been Pagan). Alternatively, if you are a vassal you can only use holy war if your religion is from the same religious group as your liege's religion.
That makes it more curious, then. I was Miophysite (horrible luck down there in Ethiopia nowadays) and so was my liege, or at least should have been.

Now I'm wondering if the fact my capital got relocated had any effect, if new Jade Dragon CBs perhaps overwrite holy war CB in case you're trying holy war rest of your capital duchy... hardly. And I didn't see new duchy CB either, just border dispute CBs. Duchy in question already had holder, anyway.

That particular example looks like a bug of some kind, but if you can create a title you should get a notification saying so at the top of the screen.

How AI/Player can create titular titles like this?
No de iure lands.

Custom kindgom/empire titles feature in Charlemagne DLC.
Kingdoms exist and/or no duchy fully under control, thus titular.

Should I report a bug?

As i said, it's probably no duchy fully controlled or the kingdom titles of their territory exist already, so consequently no de jure territory for the custom title.
Working as designed.

Name of title i really strange. I don't know how AI get it.

Oh..yeah that sounds strange.
That could indeed be a localisation bug of sorts...or rather an AI custom title naming glitch/oversight.

Sometimes the viceroyalties I have given to my vassals become regular kingdoms. Why does that happen?

Please no.
Assuming you use no mods that influence it, that sounds like a well known bug has returned.

Nope, pure vanilla.
Honestly, I expected some faction/inheritance sheningans being the reason here. The viceroy was a lunatic, possessed, cruel maniac and his son, who became the king somehow, was not an angel neither. Loads of martial and intrigue. The viceroy died on some disease, though, and there are no demon worshippers in my realm because of buddhism, so I thought it maybe happened during some civil war or something. I remember requesting peace in that kingdom around the time that could happen.

.....so I thought it maybe happened during some civil war or something....

Even then it would still be a bug, wouldn't it ? That was also what i remember when the bug happened.
First time a wrong succession law and issues surrounding that and the second time the bug occured
because of the title becoming standard after a rebellion of sorts....or so i believe to recall.
Well, ...i don't know. Your business. :p

Just looking around my vassal's holdings I notice that sometimes the AI is doing upgrades, other times it's not but will let me do them. This rings a bell from Stellaris i.e. that the Liege can do upgrades on vassal holdings (as with constructions in Stellaris Sectors) if there is a chance to do so i.e. the AI is doing nothing and the upgrade button is green. Is this how it works?

You can build holdings, and improve holdings inside your realm, so in your demesne and in your vassals territory.
That's it. Not when, what, where or if.
You can just do it.

If they have the money they usually build stuff. Barons can lack the means to do so as they don't have a lot of income. I usually jump start new cities by building some of the money making buildings for them

Forgive me if this is the wrong place to post; I'm new to the forums...
Is there some sort of new loss condition? I can't make it past two characters before I summarily lose, even with an heir, for no apparent reason. My character doesn't die, my title hasn't been revoked (or, at least, if either of these are happening, I'm not being informed before receiving the screen telling me that it's game over). All I see is that I've faded into obscurity. This is my favorite game, I'm excited for all the new stuff, but something is making it unplayable.

I'm on the Liege Council of the French King (Marshall) and am forever being asked by fellow councillors to support them. However, I've not yet managed to work out what they want me to support. Is it possible to see this info anywhere?
