If you're working on portraits, can you add some eye cover that appears on blinded people after a few days/weeks? I'm not sure how pleasant staring at eye sockets is going to be if it happens to your ruler.Note that the new portraits you see are part of a seperate DLC, as I think Doomdark has mentioned.
I mean, say you capture the current emperor in a war (happens sometimes). Instead of just releasing him, he gets castrated.
