So the medium crown authority says vassals cant wage personal wars in the realm... But as Byzantine Empire, i have medium CA and my vassals are engaged in an eternal war between them for counties, etc... Its stupid I, as Basileus, can't order them to stop this nonsense.
And the worst thing is that these stupid petty wars have no end, because the stupid counts attack each other with ridiculous armies that cant lay siege to a holding, so they stay there forever... is just stupid really, must be a big bug.

Vassals can't press claims against each other under medium crown authority, but they can end up at war because of plots. Also, it's possible that some of your vassals don't come under your crown authority if they are outside of your de jure area. If you hold multiple kingdoms, make sure that you set the crown authority in each kingdom. If however, some of the vassals are de jure part of another empire or kingdom then they may be under another character's crown laws and in that case there isn't much you can do.

You will never have the authority to prevent all wars in the realm. The reduction in claim wars that Medium Authority brings is already extremely significant. You need to learn to deal with other characters having similar ambitions.

Having said that, I do think that all wars should have an increasing war score for deadlock situations. I'm not sure why some types of wars don't.
