I had a game where a CoT appeared out of nowhere in one of my province (there wasn't even a notification). I didn't build it, and it's not even my NF. And after a while the CoT disappeared. Any idea what's governing these?

If there are fewer CoTs than the game thinks there should be (which is based entirely on whether the number of provinces in the world divided by the number of CoTs in the world is above a certain value), CoTs will spontaneously appear.

If a CoT has too low a value, it starts accumulating Stagnation. At 50 Stagnation, it disappears.

There's a whole thread which includes this mechanic in my sig.

Basically, if there are more than 30 occupied provinces in a continent then a new one will be spawned. Cots above 300 in value tend to be stable. Any smaller and they will stagnate. Sendng merchants reduces stagnation. At above 50 stagnation, the CoT (should!!) dissappear within a few months.

New CoTs will spawn wherever they want. They can spawn next to existing CoTs (even though you cannot build one in such a place yourself). CoTs are not able to auto-spawn in a province which belongs to a trade league.

Small quibble: it is actually the total occupied provinces in the world divided by the number of CoTs in the world. Where those provinces are located is completely irrelevant to the triggering mechanism; i.e. colonising 5 provinces in each of the 6 continental areas or 30 in one will still result in an additional CoT spawning (if one isn't built).
