The Mayans just need to be taken over completely. Use a diplomat, offer peace, and choose annex. You know that you have complete control if the peace score is +100%. Otherwise you are missing something. Unless you choose to offer peace, you won't get the annexation option. Peace treaties are how you get things in this game - seizing them isn't enough.
If the Mayans have converted to Catholicism (it happens...) you have to treat them like any other non-pagan and dismember them a few provinces at a time.
Forced annexations only work on one province non-pagans or pagan states. In the first EU, you could annex any country you completely controlled, but not in EU2.
