Conditions of Plot to Revoke?

I am quite confused on what the requirements of the "plot to revoke" intrigue action.

The pictures is a start as the Petty Kingdom / Dukeship of Mercia in 769. Why is it that I can plot to revoke both titles of the Count Worchester and Warrick but only 1 from the Count of Derby and Chester, and none from the Count of Oxford and Bedford?

Are you suppose to be only allow to plot to revoke titles from one level below you? Like kings can only revoke from dukes, dukes can on revoke from counts, and counts can only revoke from barons?

For starters, those with demesne of 2 and more. Then counties in the same duchy as your capital, as long as you hold the title. After that, not sure.

Counts with more than 1 county, Dukes with counties outside of their dejure duchy (only if they personally hold the county, if it's held by a vassal of theirs, you don't get the plot) and counts inside the duchy your capital is in AND you hold the ducal title of

Did something change with the patch? I know the answers given in this thread used to be accurate, but in my current game I'm 100 years in and haven't yet had the option to plot revoke once. My only plots are murders.

Also, Content characters cannot do it. Might be other traits blocking that plot as well.

Can't have a non-aggression pact either.

EDIT: On a related note, you can revoke all titles from a vassal for free if you first plot to revoke a county, he rebels, and afterwards you use that as a reason to imprison him (as long as you don't immediately succeed in the imprisonment).
