Kept inviting people to court to make them commanders, they didn't show up in the list, finally spent 71 gold to use intrigue decision to promote commander, he doesn't show up in the list either. Something this basic and important shouldn't have complicated hidden rules.

Yeah, there aren't a lot of "complicated hidden rules" as to who can be a commander. Basically any non-landed character in your court or any ruler who is a direct vassal of yours can be appointed. About the only restrictions are gender restrictions or if you have the box for a councilor checked that he can't be appointed.

Sounds like the Byzantines to me. Quoting the wiki: "Also, only characters with a title higher than a baron may become commanders (unless there's not enough counts/dukes/kings in the empire to fill all commander slots). "

That's the only case I can think of where there are special rules about who can be a commander.

Only other possibility for "random invited courtiers can't be commanders" is playing as a Hindu ruler, where Hindu commanders must be of the Kshatriya caste. This shouldn't ever face an issue with the intrigue decision though.
