Two example for small decissions that would add flavour imho.

Historical Backround: In HRE the succession allways was a hard fight so sometimes it was decided to give the title of a Co-regent ("Mitkönig" / "Mitregent") to
the son of the current King. For example the upcomming Henry IV in 1053. This title had
no imediate effect but it was a boost in prestige.

How to implement?:
As decission

Conditions:
- Be independent
- Have a heir

Effects (when not electoral monarchy):
- Gives prestige to the next person in succession + improved relations
- Costing little gold (ceremony)
- Chance of seriously damaging relations to the 2nd in the succession line

Effects (when electoral monarchy):
- Gives prestige to your son
- Costing little gold (ceremony)
- Chance of seriously damaging relations to the biggest duke/count

What would it add to the game?:
Everyone wants his new ruler starting with high prestige but it
could be the start of a civil war to support your son/successor too much.

Historical Backround:
Becoming King is not only a pleasure, it's also very expensive!
And a King without a propper coronation would have to face serious
trouble. There are even examples where kings made debts to pay
for their coronation fesivities because it was so important.

How to implement?:
Trait "uncrowned king" giving relation malus to every vassal.

As decission

Conditions:
- King/Imperial level
- Hasn't had a coronation yet
- Has X money

Effects:
- remove the modifier "uncrowned king"
- Lose X money

What would it add to the game?:
A downside to becomming king and if there are many kings
following in a short timeframe they won't all have the money
to aford the right ceremony or didn't care like in the historical Interegnum
in the HRE where Kings where elected that never had a propper
coronation (William of Holland, Alfonso of Castilia, Richard Cornwall).

In general I like the idea. The Capetians did this at least until Philip II so it definitely fits the period. Maybe it could serve to increase crown authority or give the designated heir a better chance at being elected, since one reason for doing this would be to keep the dynasty on the throne.

I like the idea, but not having a lavish coronation shouldn't have a negative effect. History is full of claimants rushing to get the crown and be crowned properly, often in rushed ceremonies. Not grand affairs. A grand coronation might improve relations with the people, but not necessarily the vassals.

Well think I think should applie the the Holy Roman Emperor since travel to rome to crown EMperor fo rome, until then he just King of Roman.

I also thought about this but this would be very hard to implement. It would mean that the HRE title would be
not in existance for a while creating a problem if you have king ranked vassals (like Bohemia). So it would be
determinated that everytime the Emperor dies the King of Bohemia will become independent ^^
It's just to hard to implement with to little gain for the game I guess.

Well IRL the king of Bohemia was a vassal of the empire and not a personal vassal of the emperor; and there still remains an empire and kingdom even if it temporarily is without a ruler. Besides in case of the HRE, there were two imperial vicars (elector (duke & count palatine) of Saxony (parts with Saxon laws) the elector (and count palatine) of the Palatinate (parts with Frankish laws) as caretakers during an interregnum (death of the last monarch and election); and the king of the Romans could rule the empire too, he just wouldn't gain the prestige for being an emperor and not being an emperor also prevented the election of a successor during a reign.
