Something weird is happening in your video system:

This just makes no sense, and I've never seen it before. Can you run dxdiag again please, but click on the "run dxdiag64" button in it. The write out a new dxdiag.txt and post it here please (only from the top down to [sound devices] thanks).

The other thing I notice is your video driver is newer than the latest official release, which was in September. Did you install a beta version, and if so, why?

What I suggest is that you manually uninstall your video driver, and reboot - it will use the default Microsoft driver. Then install the latest official driver from AMD
