I own EU III, EU IV, Victoria II and CK II. These all appear on my profile. However, I also own all the CK II expansions (exception: the Aztec one) and these do not appear on my profile. How do I change this?

By profile, you mean appear as icons under your name here? You have to register them, just as you have the base game.
You don'r have to, they perform no function - they are for show only. Only the base game registration actually does anything.
In another thread, I ran into a poster that had the Byzantine Empire icon on his profile. I have the Byzantine icon on my profile, but it is not the empire-tier icon. How do I get that?
Not sure what you mean - the Holy Knight icon perhaps? Collectors Edition?

An unrelated question: do the CK2 DLCs also come with instruction manuals? Or just the base game? If they exist, where are they?

People who pre-ordered CK2 initially had exclusive access to empire and kingdom tier avatars. This was later removed, so the only way to have them is if you set them as your avatar before they were taken away and haven't changed it since.
Not a question for this forum, really, but the ones that have manuals are in the CK2 FAQ forum.
