Two of the most important effects, long-term, of the Black Death were the decline of feudalism and the rise of the reform movement in Catholicism.
It would be nice to see high Unrest in Depopulated provinces, and to still have a lower Unrest penalty until/unless the player chooses to ease up on the serfs (decreasing taxes and levies permanently, but not by as much as before). Depopulation should also cause Moral Authority for the religion of that population to fall.
There was also quite a lot of variety in how much damage the Black Death caused; some Chinese provinces had 90% death rates, some Italian cities had 75% rates, and some English counties had only 25%. Depopulation should occur in phases, and you should be able to prevent it from worsening by investing resources in stopping the spread.
