Please explain the issue you experienced in the most condensed way possible
On the CK3 Mod Coop discord, we've been experimenting with on_actions and appending to them as you can in Imperator. on_game_start, for example, doesn't allow you to append to the on_action in a new file with a new name. It instead, overrides the on_action for on_game_start entirely. So for now, the only way to adjust these on_actions, is to override the base game file. With that said, I could append to the birthday on_actions without an issue.

Please explain how to reproduce the issue
Create a mod that attempts to append to the on_game_start on_action.

Is there anything else you think could help us identify/replicate the issue?

Would you mind mentioning how you got it working? It not being clear how to append to on_actions looks to be a significant issue for mod compatibility.

And this does not replace the existing event and effect values and allows multiple distinct mods to append to the same base on_action?
It seems like effects get replaced, but on_actions are appended.
