I'm playing on 1.4.4 and I've just formed Portugal as duke Nuno but achievement didn't fire however, I had become king of castile first through meritocracy, could this affect the outcome?

The description for the achievement is wrong, you have to do it as Duke Nuno himself not a descendant.

Hello grisamentum!

Thank you for your report! Just as logros13 said, you have to form the Kingdom as Nuno himself. We have added the wording issue to our bug database!
Hello, just replying to this to let you know I had the same issue as OP. After reviewing this thread it appears the achievement didn't fire since I formed Portugal as Nuno's son. I was just hoping the ambiguous achievement direction could still be fixed. It's worded the same as other achievements which allow descendants to obtain the achievement but obviously this one does not.
