Please explain the issue you experienced in the most condensed way possible
Starting as tribal count. Tribal duke liege's title revoked by feudal king overlord. Feudal king became direct lieg (without me feudalizing). I suppose this was the trigger for a bishopric and a city appearing out of thin air. See attached save game.

(Subsidiary, the holding of Târgoviște should be renamed, the county with the same name is a neighbour)

Please explain how to reproduce the issue
Start tribal. Have your tribal liege be replaced by a feudal liege without feudalizing. Feudal holdings will pop-up, similar to the chance they have to appear when feudalizing, ref patch notes.

Is there anything else you think could help us identify/replicate the issue?
