Please explain the issue you experienced in the most condensed way possible
I rule as king of sweden, routed from björn. Problems first occured when i played as the third and fourth character. I now have the whole scandinavia, along with Brittain, france, and big parts of spain. The problem consists of duchys created, gets uncreated over and over again. I think its mainly the once in britain, france and spain... Not in scandinavia.

Please explain how to reproduce the issue
Create a duchy wait a couple of mounths and they are uncreated...-.

Is there anything else you think could help us identify/replicate the issue?

If this isn't a bug, then the most likely explanation is that your vassal dukes are losing their last counties in wars with your other vassals. When this happens, any titles that the now-landless character held (and which weren't part of the war claims) are destroyed, as a character can't hold a title without also holding some land. Solutions are to increase your crown authority and make sure sure that your dukes hold enough land to defend their titles against your other vassals.
