Please explain the issue you experienced in the most condensed way possible
My Player Heir is listed as my daughter Mor, the first born child by my wife. However, I have older sons by my concubine, and younger sons by my wife, and none of them become my Player Heir.

Please explain how to reproduce the issue
Start with a culture with the Concubinage tradition but a Monogamous faith (like the Catholic Gaels). Have children by a concubine, then by a wife. Reform or hybridize the culture to remove the Concubinage tradition. The first born child will become and remain your Player Heir, whether or not the child should inherit anything.
