Short summary of your issue [1.1, Achievement Ironman] Holy Order requests + pays for holdings outside my domain, receives nothing in return.

Please explain the issue you experienced in the most condensed way possible
As a Lollard King of East Francia (the only independent Lollard ruler for almost all of that time), I founded a Holy Order in one of my holdings. In the century or so since, they have repeatedly asked for the lease on various cities, outside my personal holdings, via the usual "We'd like a place." event. I'd been accepting the money on the assumption that they were getting stuff off my vassals, and hey, I'll take the cash. But I saw they weren't getting stronger, so I looked and they don't actually own any holding leases except the one I founded them in. I played until they next asked for a holding, accepted the cash, and immediately checked - no city was in their possession.
Please explain how to reproduce the issue
Either play on until the holy order masses money and asks for a holding, or use console trickery to give them the money / trigger the offer. I'm not sure if this will happen in any founded holy order, or is particular to my situation.
Is there anything else you think could help us identify/replicate the issue?
I suspect that the event isn't checking if I actually can give the lease on a holding before it makes the offer, and then when it tries to give them the holding it's noticing I can't on account of the holding not being mine, and not giving it to them, but by then the money has been exchanged.
