Please explain your issue is in as much detail as possible.
If you are the only Feudal Kingdom on the world map diseases & epidemics will continuous spawn throughout your realm.
Game rules were even set to Reaper's Due fewer epidemics & outbreaks and it felt like I was playing on Deadly+

Steps to reproduce the issue.
Begin a Random or Shattered World start with the government type selected as Nomadic and/or Tribal and all characters as single county counts.

Conquer to form a Kingdom

Reform your kingdom as a feudal government (being the only one to do so on the world map)

Watch epidemics & outbreaks spawn continuously in yours and your vassals demesne while the rest of the world sits by and laughs at your horde of dead peasants from recurring minor epidemics such as slow fever, measles, camp fever etc.

I just did a fresh game to re-verify the findings (video coming soon) and given this was just in a quick 10 year period and with the rule set to have FEWER such outbreaks the outcome when extrapolated and played out longer is devastating. The entire map outside the player realm had 3 slow fever outbreaks whilst the player suffered what appears to have been 7 concentrated biological terrorist attacks (3 slow fever, 2 measeal and 2 smallpox).

The same problem occurred for my trade republic. Spent 80% of the time locked inside the castle.
