Please explain your issue is in as much detail as possible.
So according to my understanding of holding multiple titles, some that are elective, and some that are primogeniture, is that each title would be inherited independently, but it seems that that is no longer the case. I decided to go and play an ironman game to get some achievements before all the mods update. I was Emperor of Byzantium, and had a single under-aged daughter. I couldn't get her to be voted next Empress, but was confident that she would inherit all of my duke and county level titles, at least one of which was Agnatic Primogeniture. There was an alert saying I would lose the game, but put it down to a bug, and tried to get a son out to resolve that possibility.

When I died without a son, however, I lost all the titles, and thus the game (Note: after reloading the game, I got a son at the last minute, and still lost the game). She was definitely born legitimately, and was definitely of my dynasty. I checked the inheritance rules, and it said they were elective (even though I never changed that), but at least one duchy was completely controlled by me (so I would have been the only elector), and there was no way for me to select who I would vote for for the lower-level duchies, but by default I thought it selected your biological heir (Note: after re-opening the save I found out that the heir for all my titles was the expected successor for the emperor). I managed to lose the Emperors crown once before without losing my ducal and county titles (my saint great-grandfather was emperor, but my grand-mother didn't win the election).

What gives? The only thing I can think of is that in the previous election I lost, the winner was already landed. In this situation, the winner was unlanded.

This was regular elective, not imperial succession. It became regular elective after the council became fully empowered about two generations before my great-grandfather, and much less than 100 years into the game.
