Please explain your issue is in as much detail as possible.
Playing as Count Petros of Tortosa, Middle Ages bookmark. In March 1068, barely into the game at all, I got a game over with the game declaring I lost my last county. Except I didn't. No one even has a claim on my county. On the very day, a Duke next door usurps the County of Alexandretta, but that has literally nothing to do with me.

Quite honestly, I have had NOTHING BUT TROUBLE trying to play Paradox games lately, with bugs around every freaking corner. The game doesn't even tell me what happens in this case! This is utterly stupid. When will Paradox get around to fixing the hure back catalog of bugs in their games?

Pretty sure what has happened is you liege - a duke - lost his last county. Which means he immediately usurps a vassals county. As far as I know that is intended and not a bug.

If it's not an ironman save and you are his only vassal, declare any kind of war on your liege before he looses his own war. You might not game over. Even if he can get some other county and war will continue, he should not have any levies to contest your armies.

Double checked and nope that ain't it. On the day the Game Over happens, NOTHING else happens except my Duke usurps Alexandretta from a guy who has a very similar name but is not the same person. My Duke's title isn't even being contested in any way! The ONLY war in the realm is for Armenia, which has nothing to do with me or my duke. It looks like the game mistook me for this guy with a name close to it. But I can confirm that what you stated is not what happened. Again, it's a bug. A game breaking bug. And yes I'm ALWAYS on Iron Man because of Paradox's absolutely stupid fascist rules and control over achivements. In fact, Paradox has me HATING ACHIEVMENTS NOW because of how bugged their games are with Ironman. I haven't been able to finish a single game in ANYTHING from Paradox in MONTHS because of rampant bugs that they're just not interested in fixing apparently (for HOI4 they keep telling me to reinstall every time I have any bug). And yeah I just looked again to confirm. NOTHING happened to my Duke or the chain of command for my county. It's a bug.

Between these bugs and the horrid load times (CK2 takes 10 minutes to boot up on my laptop, which is a VERY fast laptop I might add; EU4 takes almost as long, and HOI4 takes like three minutes), I'm about to just uninstall everything from Paradox and quit playing. This company used tyo be my absolute favorite, but lately their DLC and patches all come riddled with bugs because they rush things to market. I'm tired of the need for speed in releases in the world today. Hell I'm sick of everyone rushing everything. Tired of it.

I miss when games weren't released until they were 100% finished and cleaned up.

So I looked at the same game and...

Not a bug, it is an invasion. Winning an invasion will give you the area you are claiming and any other counties that have been occupied.
