Please explain your issue is in as much detail as possible.
'A' has son 'B' who has sons 'C' and 'D'. Age of 'D' < age of 'C' < 16. 'A' dies, 'B' inherits even if 'D' is designated heir. 'B' dies, 'C' inherits even if 'D' is designated heir (both 'C' and 'D' still under age).

Steps to reproduce the issue.
Start as Orseolo in Venice in 769, let your son have a second child, try to designate him as heir.
