Following the successful first crusade, my beneficiary was installed by the Pope as the first Christian King of Jerusalem. However, in his bloodline, he's called "Sultan." See screenshot below.

Is that working as intended?

Is it possibly related to him culture shifting near-immediately to "Levantine"?

This is pretty much what this is. Levantine rulers are titled Sultan so when he converted culture his title changed.
