All of the above

Please explain your issue is in as much detail as possible.
While playing from the Erik the Heathen 1066 start, a woman whom I had captured (a daughter of William the Conqueror) was designated as a Crusade beneficiary by her brother, Richard, the current king of England. By the time the Crusade ended, this woman was a concubine and had converted to paganism. Nevertheless, she was awarded territory in the Holy Land after it was won by the Catholics. Oddly (related bug?), the crusade ended with no primary beneficiary so the Pope took the lands for himself, but he still ended up with a pagan duchy-level vassal... who, for a single day, remained a concubine before being automatically removed from that relationship.
In short, the new Crusade beneficiary designation system fails to disqualify 1) prisoners, 2) concubines, and 3) non-Christians. Given that concubines are otherwise disqualified from inheritance, this seems to be inconsistent with the game's internal logic.
Steps to reproduce the issue.
1. As a pagan who practices concubinage, capture the female relative of a powerful Catholic ruler.
2. Wait for a Crusade to get underway.
3. Take the female relative as your concubine, and convert her to your pagan ways.
4. With luck, watch your pagan concubine abandon your court for her new pagan fief in the Holy Lands.
