Please explain your issue is in as much detail as possible.
Byzantine Empire ignores heir designation mechanic when succession law not on Imperial elective.

Steps to reproduce the issue.
Steps-to-reproduce:
- Reform a pagan faith
- Pick meritocracy doctrine(allows heir designation)
- Swear fealty to the Byzantine Empire
- Force gavelkind succession through factions or have ruler change it in exchange for lifting excommunication
- Become the Emperor of the Byzantine Empire through factions, pressing claim or inheritance.
- Change the succession law to Primogeniture
- Appoint your preferred heir

Expected result:
As all conditions for heir designation are met the designated heir should take precedent.

Actual result:
Sons of deceased firstborn son are in line before the designated son. Seems to ignore heir designation altogether, follows primogeniture as it should.

This seems to only be an issue with the Byzantine Empire. Empire of Italia and other held titles do take into account the heir designation mechanic.

Maybe it's due to the fact that you switched Byzantium to Primogeniture *after* your eldest son had 5 children and died? I agree that Meritocracy should overrule Primogeniture, but perhaps the tiny amount of time that the game sees you are Primogeniture before accounting for Meritocracy allowed the situation to happen.

I agree that this is probably a bug, though.

What is the behavior when you actually click the "Designate Heir" icon? Judging from your third picture, it looks like you have not done so. Does the icon, instead of becoming the larger crown indicating your heir, revert to a blood drop? If so, then I believe that this is not a bug but a quirk of the purple-born heirs mechanic.
If you have purple-born heirs, primogeniture succession, and heir designation, and you want to designate a non-purple-born middle child as your heir, then you first have to give that child the Despot honorary title before you are allowed to designate them.
Try giving him the Despot honorary title and then clicking the "Designate Heir" icon.
