Please explain your issue is in as much detail as possible.
Seems to occur during most regencies, when the council votes against anything it insists there's an empty seat even when all council positions are filled, see attached screenshot.

Steps to reproduce the issue.
Play during a regency and attempt to take an action the council is voting against (handing out land, waging war)
(Untested theory, regent title held by a council member rather than a separate individual?)

I've seen this, but I have not seen how to trigger it. It does not seem "persistent" to me, but rather intermittent. If you have a save game where this is happening, try saving and restarting (to check the problem persists on a new load) and if it does, I would say upload that save game here, as I don't think it's as easily reproducible since the trigger is unknown.

One thing I suspect- in your screenshot is your regent one of your councilors? I've been playing on a shattered world start recently with all 0 yo counts, and I NEVER start with the issue active, and in those cases the regent (who gets a vote) is not on the council. The game may be treating the regent as an empty seat.

I noticed the same thing earlier today when I was trying for the Lord of the Flies achievement.
I am convinced it related somehow to the regent since usually that's when I'd find the council interfering with my plans, also encountered it prior to Holy Fury's release
