On this run, my first character died almost immediately; his son (Frirek Jute) took over at age 5ish and had the "Chess with Death" event in his 30s. As a result, he lived to 90-something. (When he eventually lost the modifier to cure his food poisoning, he died a few days later.)
During this very long life, he managed to forge two bloodlines - one via the ambition, and another via the wolf warrior legend event chain. Much later, I noticed that every character that had any bloodline also had this bloodline. It looks like the bloodline has been added to every character (historical or not) that has ever forged a bloodline. (Yes, even Karen in AD 70.)
Notably, the wolf warrior bloodline has been added to characters who forged a bloodline after the death of Frirek - so the problem may continue to recur for as long as I play this save.
My best guess is that the game gets very unhappy if a single character manages to forge more than one bloodline. However, it's conceivably possible for one character to forge 4 bloodlines*, so the game really should be able to handle this elegantly.
*Specifically Arpad in the 867 start: forge a bloodline via ambition; then settle in Carpathia; then forge a legend in your warrior lodge; then get sainted/worshipped after your death.
Steps to reproduce the issue.
Load the attached save. Note that everyone who has a bloodline also has the wolf warrior bloodline.
