Please explain your issue is in as much detail as possible.
So I've been playing as the Makedon Dynasty and I've been noticing both a worrying lack of sons and a tendency for characters to have fewer children - to the extent that I've literally gone through three wives in ten years and then only had three daughters with my current character.

That might be one character having low fertility, it's not his wives because he seems to have been cuckolded each time.

It seems like the problem might be getting worse as the game progresses, though. it doesn't seem to be as much of an issue for smaller Dynasties but that's just anecdotal.

Something is definitely different, though, and it's making life a lot harder than it was previously. I would humbly say it's getting bad enough for me in this current game after ~250 years that it's becoming a detriment to enjoyment.

Strangely, wife no 5 (or 6, I lost count) finally started birthing sons and I now have five.

This makes me suspect that in CKII it is the woman who determines the sex, not the man. I know they made some changes to Pregnancy with the patch.

Ive had many games when i get 15-20 daughters, and after finding new wives/conubines ill get a son or 2 after another 5 daughters.

Hmmm, normally these types of complaints can be attributed to confirmatiion bias, but I've noticed this in my games as well. I started about 4 games with pregnancy event spam to pump out as many children as possible (usually about 15 children per game), and in each case daughters are the significant majority.
In real life, the birth ratio 107 boys to 100 girls, since more males die due to unnatural reasons (combat deaths for example), humans evolved to produce slightly more sons to compensate.

I didn't keep track of any statistics, but it did seem to be an strange proportion of daughters to sons to me while I was playing. Glad to know it wasn't just in my head.

I haven't noticed any issues with fertility though.

This happens to me too. Too many AI dynasties die out because they simply cannot get a male heir while freely marrying off their inheriting daughter patrilenealy.
