Please explain your issue is in as much detail as possible.
After landing an eligible man to the county-castle holding in Wiltshire, his government type and portrait indicated that he was the holder of a city.

He is also giving his liege the "Wrong Government" relationship penalty.

Steps to reproduce the issue.
I am not sure how reproduceable this is.

Just to confirm, I've been getting the same problem (under the same settings) - rulers that should be feudal (or Iqta) sometimes seemingly randomly turning into Mayors or Theocracies (not when Iqta, of course - but I've even seen a tribal county grant become a theocracy) when getting granted a Castle county. It's unpredictable and highly annoying.

The issue even seems to happen to the AI sometimes, although I don't have any pictures of evidence at the moment.

Edit: Evangeline gave a pretty good explanation on the bug here. But aside the listed cases, characters with claims of the wrong holding type (probably a former mayor in my case) also seem to be affected.
