Please explain your issue is in as much detail as possible.
Sometimes I get event that one direct vassal usurped titles from another one and in the process a barony in that county can get independent from the realm
Steps to reproduce the issue.
1)Load save
2)Find county of Biskra - it has city Magra - it's owner is a vassal of county level bishop which is vassal to duchy level priest which is vassal to the emperor.
3) Let game play for few days - county will be usurped by hellenic priest but city of Magra will become independent from the Empire.
