Please explain your issue is in as much detail as possible.
I started this game in a Steam copy of CK2 on November 16, 2018, in whatever version was latest at the time. I updated as soon as the patch was pushed through Steam.

I started this Ironman game as the Abbasid (Arabian) Empire. I went Messalian for a while. I finally converted to Zoroastrian. I reformed the Persian Empire. I reformed the church. The Moabadan-Moabad declared a great holy war for the Kingdom of Africa and I won that handily and took the titles going with it. And then a long while later I realized that I was no longer seeing any great holy wars, and I couldn't divorce my spouse essentially for free anymore.

Either to the first or the second holder of the church title, I granted two counties. Another of my vassals attacked him and took the two counties. It may be that this is relevant, or perhaps not.

The title has only ever had 2 holders, and it's been vacant since then. (It just so happens that this guy had only 2 holders of the title in his game too: https://www.reddit.com/r/CrusaderKings/comments/7k290b/how_do_you_rerestore_the_zoroastrian_church/.)

Steps to reproduce the issue.
No idea.

I suspect now that the second church title holder (the one that destroyed the title) is the one I gave the two counties to. I gave the counties to him in 1216. He destroyed the church title in 1222. He never lost the counties until he died in 1226 of an infected wound, and those counties were inherited by election.

I realize there are reasons to destroy religious leader titles (I myself destroyed the caliphate earlier in the game). But once the title is activated by a decision, this title should not become inactivated without a very good reason. Especially if the user spent a lot of time and effort in establishing the church. If the church could evaporate so easily, the game shouldn't require players to put so much effort into it, and shouldn't encourage players with bonuses like essentially-free divorce and great holy wars to achieve it.
