Please explain your issue is in as much detail as possible.
I'm a fair way into a Hispania run. I took Imperial authority fairly soon after becoming an emperor, and I used every available means to transofrm all of my hereditary ducal vassals into minor viceroys. More recently, I had to start creating kingdom viceroys to stay within my demesne limit. That's when the weird AI behaviour started.
My first kingdom-level viceroyalty was Jerusalem. I didn't own the county of Jerusalem until shortly after creating the viceroyalty, so the viceroy's capital county is Acre. Ever since she became overlord of Jerusalem, she's been continually revoking and granting the county. The only explanation I can think of is that there might be two AI directives acting in opposition to each other: (1) own the kingdom capital; (2) don't have more than one county in your demesne. #1 is expected AI behaviour. #2 seems unsupportable - but a similar effect has occurred with one of my other kingdom viceroys.

Sicily: Owned Naples for a long time; then Capua was conquered and was granted the minor viceroyalty of Capua; then, when all of Siciliy was conquered, he was granted the kingdom viceroyalty of Sicily; he almost immediately gave away the county of Capua. (Naples is the de jure capital of Sicily.) He now has one county (the de jure kingdom capital) and a random barony (in the island of Sicily), has a large demesne limit, and has shown no intent to expand his holdings.

Counterexample - Egypt: Granted Cairo and Faiyum on the same day as the viceroyalty of Egypt; hasn't disposed of the counties, or any of his additional baronies.

There's definitely something wrong with Jerusalem - she shouldn't be ping-ponging between revokation and granting. There's probably something wrong with Sicily, and it may or may not be related. But Egypt shows that whatever's wrong is not universal. Erm... good luck?!
Steps to reproduce the issue.
Load the attached save. Kill the count of Jerusalem (so the viceroy of Jerusalem inherits). Run the game. Observe that the viceroy of Jerusalem will grant the county to a random unlanded courtier and then, later, try to revoke it.
