Enatic Clans enforces either enatic or enatic-cognatic on all playable governments plus theocracies and is implied to do so completely globally. However, regular republics seem to be an exception and carry on with standard agnatic open elective. Cities handed out later will initially have a female mayor and have enatic-cognatic open elective, but this is in practice actually identical to agnatic open elective and all successors will be male.
Steps to reproduce the issue.
Start as any Pagan ruler and reform with the Enatic Clans doctrine, observe cities under you.
