Please explain your issue is in as much detail as possible.
The "Introduce Heir to the Realm" decision is supposed to be the final part of the "Groom an Heir" ambition - and the extra stats and vassal opinion can be very useful.
It can already be somewhat difficult to take this decision in some types of succession because it requires your non-adult near-oldest son to be your heir. (Eg. in elective succession, electors sometimes don't like to vote for children, so your eldest son may not be your heir until after he becomes an adult.)
However, if you are a Doge (leader of a merchant republic) then it is literally impossible to use this targeted decision. The reason is that it requires the target to be non-adult and also the heir to your primary title. Since your primary title is a MR and a MR can never be inherited by a child, this means that the targeted decision can never be taken.
I believe that players who play MRs spend most of their time as the leader of the MR. Therefore, they will be almost permanently locked-out from this feature. The elective succession thing isn't ideal either, but it is at least understandable.
Suggested Resolution: "Introduce Heir to the Realm" should be available if any of the following 3 conditions hold:
- Is the heir to your primary title
- Your government type is MR and this particular son has the honorary title "Designated Heir"
- Your succession type is elective (or elective gavelkind or tanistry) and you are currently voting for this particular son/daughter.

Load the attached save. Observe that the "Introduce Heir to the Realm" targeted decision is unavailable - and, further, can never be available.
