When I give a duchy title to my Lord Mayor, he becomes a feudal duke instead of the Grand Mayor of a merchant republic.
I think it's related to his realm capital being a castle. At first he was a mayor in Lancaster, then I gave him the County of Man, expecting him to build a city. But apparently, when he built the city, he made the city the county capital without making it his realm capital, so he's still "reigning in" the castle.
If so, there are several issues here:
* Having a castle as a realm capital should not prevent becoming a Merchant Republic
* His realm capital should have changed automatically when he made the city the county capital
* Capital move timer should not prevent moving the realm capital within the county
