This is more immersion breaking than anything. They're in another county (usually) and presumably have their own physician on top of it all.
Simply adding is_ruler=no to the RIP.5060 triggers is all that is needed.
Steps to reproduce the issue.
Have lots of kids with titles during the bubonic plague outbreak.
