So I accidentally poisoned my only son and heir. Now his health states 'not feeling well - severe penalty. Is this permanent?

Depending upon your court physician's skill he can get better or he'll get well on his own if he has a high enough health stat.

I believe most health debuffs like this will go away with time. However, that assumes the character lives long enough, which in turn depends on if they had enough vitality to spare before the poison.

Like that of CK2, if you move your cursor to the corresponding modifier, you should be able to see an expiration time for the modifier unless it is permanent.
