I just died at 22 with the cause being "died in an accident". I've seen this happen to AI characters before but never to me.

This is someone successfully plotting to kill me and it not even looking suspicious right? Not actually some random freak accident?

Conclave added a bunch of new council events, which were things like "you start to feel dizzy; do you ask your councilor for help [and become rivals if he ignores you] or try and stick it out, hoping it's not serious [chance of injury or even dying]." Death from those events is labeled as being caused "by an accident."

Assassinations are always either "suspicious" (if the assassin got away with it) or "on the orders of X" (if they were caught). Regular accidents are never the result of assassinations, they are always the result of random events.

I never lost a ruler (or heir) in any of my campaigns by accident yet, so the chance of getting this must be either extremely low by default, or the weights are different for rulers and their familiy than for "normal" courtiers.

If you are a child ruler, your regent can plot murders against you, which will be classified as accidents if successful, as (s)he sends you on dangerous tasks, like cleaning a tower, or "hunting".

Marshal dying in a military drill accident

This must be it then. I didn't get any flavour events beforehand but I was on my liege's council at the time as marshal.

It makes sense for it to be something else other than a successful plot; if it was the result of a plot I would have seen it before considering the number of people I've murdered in this game.

There are event popups for when you are on your liege's council, and they can result in serious consequences if your stats do not provide you with many possible choices. If your ruler is dying "in an accident" and you received no event, it is most certainly a bug. If that's the case, you should post a bug report in the appropriate forum.

Note that events are shown to the player in order, like cards being dealt on a table. This results in the consequences of events being shown on top and you have to dismiss those windows before you see the reason for them. An example of this is when your ruler dies and he/she is tutoring minors. In that case, you will have to dismiss several events (e.g. your new heir, minors that need tutors, etc.) before you can actually see the circumstances of your ruler's demise.

Also, events can sometimes be delayed a few days from actually displaying. An example of this is childhood events firing just before a child's 16th birthday but not displaying until after that date. It is possible that the event responsible for your ruler's death was delayed from being shown to you. I guess it's also possible that since the ruler changes, it isn't shown to you because it isn't relevant (i.e. your current ruler isn't/wasn't on the council, so deaths related to council jobs are not relevant).

Only by filing a bug report (and providing a savegame file) will the developers be able to find out for sure what the problem is.

This is a common event. This event used to say "died of natural causes", but has since been replaced by "accident". I imagine this was because of people (myself included) complaining about how ludicrous it was to suddenly die of "natural causes" with no symptoms in early adulthood.

This is a common event. This event used to say "died of natural causes", but has since been replaced by "accident". I imagine this was because of people (myself included) complaining about how ludicrous it was to suddenly die of "natural causes" with no symptoms in early adulthood.

I have not seen this in recent builds. Instead, I've seen relatively young (mid-20s) characters die with the following explanations:

"of poor health"
"a natural death"
"from severe stress"
"a maimed cripple"
"after a period of illness"
"of pneumonia"
"of slow fever"
etc....

I've never seen a player-controlled ruler die "in an accident" without a preceding event.

That change was made several patches (= probably over a year) ago.
