Some stuff has been fixed for Cathar/Basque rulers, so I'm surprised that this is still broken.

In an absolute cognatic realm, where female rulers can be granted titles and inherit them freely, you still can't grant them minor titles. But unfortunately, if you need an extra +5 opinion from a female vassal, you can't grant her Keeper of the Horse because it seems like the gender restriction on the minor titles are hard-coded.

The grand tournament decision also seems to be similarly hard-coded.

Meh, then you get into "Is it master or mistress of the horse?" The minor titles represent a very strict societal system which for this time period placed the interests of men above all. If your realm has progressed to have an absolutely cognatic succession, these postiions are if anything probably redundant. You should be able to give them away but I don't think its broken.

Its also worth noting Cathars wouldn't approve of ceremonial meaningless decadence!

This. Catharism is extremely poorly modeled in CK2.

And then regarding the Basques: They did indeed have equal inheritance rights. But that is not the same as gender equality. Gender roles were still very-much a thing, and quite separate. You wouldn't have a female Master-of-Horse, just like you wouldn't have a male Mistress-of-Robes. Just because both could rule, did not mean they had the same opportunities as each other. To pretend otherwise just glosses over historical culture.

Speaking of which, there really should be a Mistress of Robes minor title - the chief lady-in-waiting for a queen.
