After playing 700 hours normally I finally decided to bite the bullet and try out Ironman to get some achievements. I tried clicking the option but it gave a yellow tick with red explanation mark saying that the game files had been altered and achievements were disabled.

I checked and there were no mods enabled. So I decided to delete the local content and re-download the game from Steam. Still I get the same message that the game files have been altered and achievements have been disabled.

Is there something I'm missing or is there a bug in the game?

Did you also manually delete all the local files in Steam/SteamApps/common/CK2 AND Documents/Paradox/CK2?

Its usually an edited file. I edited the file to allow assassinations and totally forgot until I went to start an ironman game.

I only had Steam uninstall CK2, I had assumed that would remove all the edited files. Does that uninstall only affect the Steam files and not the Documents files?

If it does not affect the Documents files how am I to get fresh CK2 files? Just delete Documents/Paradox/CK2 and then reinstall CK2 on Steam?

I'm a little reluctant on deleting all the mods though. Maybe just delete the edited folder? The only one in documents I made changes to was the interface.

Dont delete the files. Just change the folder name (rename it to CKII.bak or CK2.old or whatever makes sense to you). This way tou get to keep all of your files and since the game doesn't recognise the folders you get to use a brand new profile.
