I'm trying to form France, but I do not see any decision that will allow me to do so.

I am fulfilling all the requirements mentioned by the wiki...

Requirements
-Must not be Spain (a nation with Basque primary culture can form either France or Spain), the post-Renovatio Imperii Holy Roman Empire, or the Papal State.
-France and Revolutionary France must not currently exist.
-Must own and have cores on Maine, Berry, Vermandois, Île-de-France, Orléannais, Nemours, and Champagne.
-Primary culture must be in the French or Basque culture groups, or be the Breton culture.
-Must be at peace.

I started a grand campaign as France...
That would be why you can't form France.

To clarify: There are only three ways to stop being France - become the Papal State by being a Catholic Theocracy when the Papal State gets annexed, become Revolutionary France by losing to the revolutionaries, or become the Holy Roman Empire by enacting Renovatio Imperii. If you are France or Revolutionary France, then France or Revolutionary France exists and the Form French Nation decision is unavailable. If you are the HRE or the Pope, you are ineligible to form France.

So is having France be able to be formed because you could be playing Britanny or Burgandy or any other french minor and defeat real france and re form it yourself?

Yes. Burgundy is especially able to do this.
However, you lose about ten million style points for doing so. For starters, you can paint the map a much nicer colour if you stay Burgundy ;)

Various shades of blue, cyan, and bluish-purple. Except for Dauphiné, which is a pistachio colour rather similar to the Ottomans.

France enacts the Form France decision. French nationalists revolt in protest. So do French patriots. Both revolutions quickly disband when they can't decide on an enemy. French peasants revolt due to the utter absurdity of it all!

France can't culture shift.

They can however move their capital to say, London, release all their minors, and cede the rest of their provinces in a war. They will now have English culture (assuming you haven't changed London's culture to Manchu or something through settlement policies); once they get all the required cores, they can form England.

They need two bankruptcies (a.k.a culture shifts) to create Spain because the Form Spain decision can't be made by countries with tag = FRA.

Right, but the non-decision way, mentioned above, can be tons of fun in a game that's become stale. Becoming a OPM, then going bankrupt, then trying to form a new country, can be quite a challenge, especially if you do it two or three times. The reward is you get to collect lots of nation-specific modifiers.

All nation-forming decisions in DW explicitly disqualify the Papal State.
