I don't understand this tooltip! Since I took this screenshot I now have all the counties of K_Leon. Why can't I create it? It can't be because somebody of another religion has it as their primary title and holds land within it, because it hasn't been created yet and now I have all the counties. Thanks.
León has some weird special rules for historicity. In Charlemagne and Old Gods you have to be the current holder of the Kingdom of Asturias first, then you get the decision on the Intrigue menu to create León.

As StarSword said it has to do with it being created by decision the first time. To explain the tooltip the "any previous holder" part is what is blocking it being created normally, if you look at the title history you will see no one has ever held the title so there has never been any previous holders. After it is created by decision it will be able to created normally if destroyed as the title will have been held by someone in the past.
