The image says it all. I have never experienced this one, so what must I do to make them accept annexation? Relations are 198, and I can choose the option to integrate, but then I get this message:

They are heathen (Sunni, I am orthodox). I have integrated heretics before, heathens I never integrated though. If this is true I'll have to wait until they get taken over by religious rebels, which will most likely never happen
I believe you can inherit heathens but you cannot integrate them.

A nation with a different religion I don't believe can ever be integrated, but can be inherited(it kind of makes sense).
I had the same thing happen in my latest Ottomans game, got a PU with Algiers before I flipped to Orthodox and took over the HRE, but I couldn't integrate them because they stayed Sunni (obviously). I eventually managed to inherit them (in between losing the union twice due to terrible relations after annexing vassals, but they also Westernised and ate Morocco in the process which saved me pushing through all those North African coring costs) which should happen fairly reliably if you are Ottomans - you are presumably constantly blobbing so you should definitely end up bigger than them eventually. The issue for you is that Grenada has eaten big chunks of base tax in central Spain - you should still be able to inherit them but you will need to be pretty big.

Heretics are the same religion as you, but follow a different sect, branch, etc of that religion. i.e. If you are a Catholic then you consider Protestants, Orthodox, etc to be "heretics". In game this results in a -10 or -20 relations penalty.
Heathens are of a completely different religion, i.e. Shiite or Sunni Muslims are considered by every other religion to be "Heathens", Protestants consider Shinto to be Heathens, etc. This results in a -20 or -40 relations penalty (I believe, sorry if it's off).
Basically you can inherit AND integrate Heretics, but you can only inherit a nation that are considered Heathens, you can't integrate them manually, or it will result in the image that the OP got.
tl;dr Heretics (same religion different sect) can be integrated and inherited, Heathens (different religion altogether) can only be inherited, but can't be integrated manually.
