I wonder what would happen if modern day Italians decided to call their country "the Roman Republic", and the ERE was still around.
Or if the WRE and the ERE had split up for good, I'm pretty sure the WRE would have been considered the "original".

The ERE evolved very differently from the classical Roman Republic, and it's highly unlikely that at least some of this wouldn't have been different, if the WRE had remained a part. Still, at the time, the ERE was the closest inheritor.

Apart from this, it's just a label.

WRE and ERE were split up for good, they were considerer equal, but people would have referred west as "Latin Empire" (instead of crusader state) and east as "Greek Empire" (like they already did). Like it already has been agree "Byzantine Empire" was continuous not successor.

To be precise, less than 15% of Fyrom includes any realm of the ancient Macedonian state (Philip, Alexander etc). Also it contains no area of importance to that kingdom (eg Pella, Aegai, Amphipolis and so on).

They seem to be mostly Bulgarians given a new identity following the two balkan wars, since then they became part of Serbia.

Taking this for granted, the ERE would continue to be the ERE (or someother name), and not become the Roman Empire, just because the WRE disappeared.

If by Roman Empire you isolate the entity that Ceasar created then probably true, but the ERE and the WRE were both 'Rome'. And the ERE was the only part of 'Rome' that survived. Anything that excludes the term Rome should not be used to define this state.
