I think what happens is that factions force elective succession, then the ecumenical patriarch gets elected and the theocracy is implemented automatically.Have people at least figured out how this happens, or should I just keep playing EUIV for the time being?
Things obviously got really weird for you.I've seen the reigning Basileus become Ecumenical Patriarch before so it isn't just the result of a faction placing him on the throne. I only noticed because I was playing an Irish republic and suddenly noticed that Venice had vanished as a republic; the Ecumenical Basileus held Venice for a while and then handed it off to a new Doge.
I can't remember what happened to the Patriarch title after he died; for some reason, seems like he handed it off to the Captain of the Varangian Guard.

Oh sure this is OK but I get game over when I try to make the Basileus into Most Serene Doge. Honestly, I feel like I see this happening in about 25% of my CM starts. In one of my current games the ecumenical basileus has the largest and most stable Byz I've ever seen the AI put together. They are holy warring in every direction. Almost pushed as far east as India.
Viceroyalty can only be granted to feudal ruler.I believe what happens is that the Byzantine Emperor likes to grant the patriarch a theme every now and then. When the patriarch dies, the theme goes back to the emperor, but so does the patriarchy title.
Basically, the emperor shouldn't be giving the patriarch themes, and the patriarch title shouldn't go to the emperor even if the patriarch winds up with a theme.
I raised a bug for it when conclave first came out, but it's still around. It seems to happen pretty often, and is particularly annoying when you're playing as a vassal of the empire.

I'm guessing it means the Ecumenical Patriarch can inherit the Emperor title if he is somehow related to the dead Emperor?I went digging through old save files and it turned out that the situation was even stranger than I remembered.

This is Basileus Kyrillos the Wicked. If you'll notice, it claims that he was elected to the position of Basileus but, while I wasn't paying attention when it happened, I'm pretty sure that was incorrect. That's because all of his holdings were acquired from his predecessor or personally acquired after he became Basileus; he had a successful holy war for most of the new counties and acquired the Ecumenical Patriarchy five years after he became Basileus. Would they randomly elect a landless courtier, even one of the ruling dynasty (and a distant relation at that as you'll see in a minute), Basileus?

Note Kyrillos's predecessor as Basileus, Ioulianos, to the left. No male children, no grandson, no surviving brothers, nephews, or uncles. I think what happened here was that the title was Agnatic and the only viable candidate was Kyrillos since it bypassed several closer female relatives. Also note that Thekla is Kyrillos' niece since his daughter died before him.
