You misunderstood:
The duke of Diyar Mudar holds only one county - Harran (part of de-jure Diyar Mudar). He has a single count vassal, who holds the rest of the duchy (Al-Raqqah and Rahba) and a single county outside it (Karbala).View attachment 191609
I declare de-jure ducal war for Diyar Mudar. I occupy all the provinces within it, get 100% warscore and enforce demands.
The result: I subjugate the duke and his single province, but not his vassal count, who instead becomes a vassal of the Caliph and keeps all his provinces - even those within the duchy.

The way I understand it, Rahba and Al-Raqqah should pass either to me, or to the duke, and the count (now holding only Karbala) would become Caliph's vassal. Instead, he managed to keep all his provinces, even those I apparently won - how?

900 % plot power and it only has a 60 percent chance of seceding (SHOW me the MATH???????? My Priest vassal IS a heretic and a Zealot at that so I cant demand conversion. Here The math That about 6.2 % chance of success for every 100% of plot power? That is way too low. I had 35 backers (almost every Adult in my Realm) against the Bishop. His Intrigue was only a 5 If that Plot fails then all others will fail.

I think I see your problem, there aren't empires outside the three historical ones (unless you click the "Enable Ahistorical Empires" decision at game start). The map is just putting a empty placeholder empire as top title for all the kingdoms that fall outside those three. Not a bug. If you want de jure empires to cover all territory, then just click that customization decision and they'll all show up.Amended.

This reads like a childish tantrum, I have to say.
To address what appear to be your issues, however:
1) As with all plots (whether part of the mod or not), there is a maximum beyond which the plot power really doesn't matter. It doesn't ever mean "auto-success".
2) Again, as with all plots, plot power is not the only determiner of success.
3) I just tested the plot success chance at maximum plot power (which is 200%) without having a spymaster or regent as part of the plot, and it correctly showed the chance of success at 74%, the chance of failure without detection at 9%, and the chance of failure with detection at 16%. Really not sure what you're looking at.
4) If this is regarding the Fabricate Heresy plot (you never actually say, I can only assume that, since you refer to your game as if we've been watching it), that plot can only be used on a target who is the same religion as you. So the "why do I have to fabricate evidence when he IS [a heretic]" is because you are also a heretic and so is your religion head. Heretics should perhaps not be valid targets for this plot, but even so you're trying to fabricate evidence that they are not following your own faith. If this is regarding some other plot, I don't even know what your issue really is.
Other than expecting instant success, apparently, and anything different being "it's so random LOLOLOL", I guess? It's difficult to say with this kind of report. If you actually intend this to be a report of some kind, perhaps take a breath and try again, more rationally and with helpful screenshots or a save.
